Looking for an SSP or plan-of-action format?

NIST provides editable CUI System Security Plan and Plan of Action templates with its SP 800-171 Revision 2 supplemental materials. These are blank government templates, separate from Continuum’s illustrative tools.

These templates accompany the earlier baseline. Confirm that the format and requirement set fit your contract and assessment. Using a template does not establish compliance, authorize a CMMC POA&M, or determine whether a deficiency is eligible for conditional status.

Use the baseline that applies to your contract and assessment.

NIST has published Revision 3 of SP 800-171 and its companion assessment procedures. Its publication history also identifies Revision 2 and the 2018 assessment publication as superseded. Publication status alone does not determine your contractual or assessment obligations. Confirm the required revision and assessment procedures before mapping findings. Do not mix identifiers or objectives from different revisions.

The CSV examples identify a preliminary Rev. 2 mapping where used. A planning spreadsheet does not establish whether a finding is eligible for a CMMC POA&M or whether any conditional status is available. Check the applicable program and contract rules separately.

FROM ARTIFACT TO VALIDATION

Three common evidence problems, worked through.

These hypothetical scenarios demonstrate a review approach. They do not represent customer results or formal assessment determinations.

ILLUSTRATIVE EXAMPLE 01

A policy exists. Can you support the access decision?

A hypothetical engineering subcontractor uses an identity service and a separate project application. This example uses a preliminary NIST SP 800-171 Rev. 2 requirement 3.1.1 mapping; confirm the requirement and assessment objectives applicable to your environment.

The problem: The team can produce an access-control policy but cannot reconcile the project application's active accounts to its approved user register.
What the evidence cannot establish: A policy describes intent. An account export alone does not show that each account is authorized, that the export covers the relevant application, or that disconnected accounts have been addressed.
A practical next action: Have the system owner reconcile the two records, investigate exceptions with the appropriate business owner, and arrange approved account changes through IT. Record the scope, date, owner, and disposition of each exception. Do not describe this finding as a complete assessment result.
How to validate the change: Compare the updated authorized-user register with fresh application evidence and inspect the approvals and change records. Walk through selected account decisions with the responsible personnel. Record remaining limitations and who will resolve them.

ILLUSTRATIVE EXAMPLE 02

A screenshot is available. What does it actually demonstrate?

A hypothetical manufacturer has a screenshot of a security setting. The evidence index is being assembled ahead of an internal readiness review.

The problem: The image has no capture date, system identifier, collection owner, or explanation of which requirement it supports.
What the evidence cannot establish: The image may show one setting on one system at one point in time. It does not establish coverage across the scoped environment or continued operation of the process.
A practical next action: Record a controlled evidence reference, identify the setting's relevant system and scope, capture the collection date and owner, and explain the claim being supported. Determine what additional records, interviews, or tests are needed under the applicable assessment procedure.
How to validate the change: Ask another reviewer to locate the artifact and explain its scope and limitations using the index. Resolve any mismatch between the documented claim and the evidence. Protect the underlying technical material in an appropriately controlled repository.

ILLUSTRATIVE EXAMPLE 03

An action is complete. Is the gap ready to close?

A hypothetical technical-services firm has an incident-response procedure that names a former employee as the escalation contact. The roadmap is an internal planning aid.

The problem: The document owner marks the action complete after updating the contact name. The team has not checked whether the new contact understands the role or can be reached through the described process.
What the evidence cannot establish: An updated document supports a documentation change. It does not, by itself, validate the rest of the incident-response capability or satisfy every related requirement.
A practical next action: Assign the current process owner, approve the revised procedure, brief the relevant personnel, and coordinate a suitable walkthrough or exercise. Agree on the review scope and closure criteria before declaring the finding resolved.
How to validate the change: Retain the approved revision and participation record, document what the walkthrough demonstrated, and track exceptions separately. Close only the finding supported by the validation; assess broader requirements using the applicable procedures.
